Our Care Pillars

Three steady promises for families and referral partners

We make every decision with your loved one’s dignity, comfort, and story at the center. These pillars guide how we serve—from the first call through ongoing care.

Pillar 1

Inclusive care for every path

Inclusive

We accept Medicaid, Medicare, private insurance, and private pay with sliding scale options. We also welcome hospital and community referrals and coordinate transitions so families feel supported.

  • Coverage guidance and paperwork support
  • Flexible plans that honor budgets

Pillar 2

Holistic support for daily life

Holistic

We provide in-home support for people with limited mobility and/or personal care needs, along with sitters, errands, light housework, reading, and hobbies that help each day feel steady.

Pillar 3

Story-centered and personal

Story-centered

We learn each person’s and family’s story to shape care that honors routines, relationships, and trusted community connections.

Our story

Care that honors elders and calms families.

Sara’s Well Home Health began in one family’s living room as they cared for two aging parents—one facing cancer, the other living with Alzheimer’s—and tried to make the best choices they could with love and limited time.

The decisions felt overwhelming: sorting through care options, understanding billing and coverage, and finding help they could trust for quality and integrity. We exist because stories matter, experiences matter, and aging with dignity matters. We listen closely and shape care that feels personal, steady, and safe.

Rooted in dignity Care that protects comfort, choice, and self-respect.
Family-centered support Clear communication and steady care during stressful seasons.
Belonging at home A calm presence that helps everyday life feel safe and familiar.

Our care philosophy

We listen to your family’s story first, then shape care around the everyday moments that bring comfort, meaning, and peace at home.

Sara’s Well Home Health welcomes families, hospital referrals, and community referrals with a steady, relationship-first approach. Our team provides in-home care for people with limited mobility and personal needs, protecting dignity and offering calm, emotional support as routines are honored and trust is built.

Dignity & respect Compassionate presence Gentle community

Listening first

We meet with families, care teams, and referral partners to learn what matters most—beliefs, routines, favorite foods, familiar faces, and small joys. Every plan starts with what helps your loved one feel seen, safe, and supported.

Care that meets people at home

Our caregivers provide gentle, hands-on support for limited mobility and personal needs in the place that feels most secure—their own home. The goal is steady comfort, not disruption.

Care that feels like community

We bring companionship alongside practical support, nurturing connection at home and in our day out program—so your loved one feels surrounded by kindness and steady care.

Aging Parent Day Out

A gentle day of belonging, built around everyday joys.

Our day out program offers meaningful time beyond the home — a calm, steady rhythm where elders are welcomed by name, supported with care, and surrounded by companionship. Families can count on a safe setting that honors routine while opening the door to simple pleasures.

Companionship and conversation that feels like a friendly visit, not a program.

Comforting routines that help everyone settle in — shared meals, gentle check-ins, familiar faces.

A sense of belonging that supports confidence and calm for the whole family.

Support for families

While your loved one enjoys a meaningful day, you can breathe, work, or rest knowing they are safe, engaged, and seen.

Questions, gently answered

Frequently asked questions

We know families are often making decisions under stress. These answers are here to make things clearer, calmer, and more hopeful.

What kinds of help do you provide at home?

We offer companionship, safety supervision, errands, light housework, meal support, reading, and shared hobbies. We also provide in-home care for people with limited mobility and/or personal care needs, with support shaped to daily routines and comfort.

Who are your services for?

We support older adults, adults living with chronic conditions, and families who want a trusted companion or practical help at home. We welcome hospital and community referrals, and care can be short-term or ongoing.

Do you accept Medicaid, Medicare, private insurance, or private pay?

Yes. We accept Medicaid, Medicare, and many private insurance plans. We also offer private pay options and a sliding scale so care can remain accessible for more families.

How do you personalize care for each person?

We begin by listening to the person and family story, routines, preferences, and goals. From there we create a care plan that feels familiar and respectful, with flexibility as needs evolve.

What is the Aging Parent Day Out program like?

It’s a warm, social day program with gentle activities like gardening, baking, tea, games, and storytelling. It offers a change of scenery, connection, and respite for families.
